Obviously there's Torsion (Windows only). It costs a lot of money, but it's probably the best.

After that, we have, TorqueDev (Windows only), TIDE (cross-platform), and DebuggerForTorque (Tested with Linux and Windows only, but written in Python so probably tweakable for OS X). I have to patch the engine slightly to make it safe for using the Telnet Debugger, but other than that, these are all good editors to use.
